Division of Social Welfare takes responsibility for

      - Section of Social Works

      - Section of Child and Youth Security

      - Section of Community Development and other relevant assignments

Social Welfare Services are as below;

1. Living Allowance for Elderly Citizen 500 Baht per month

         Qualification of Person to receive Living Allowance

              - Full 60 years of age and above

              - Hometown in area of allowance receiving application

         Evidence for Elderly Citizen Registration are as below;

              - Copy of Identification Card 1 set

              - Copy of House Registration 1 set

              - Copy of Krung Thai Bank’s Saving Account Passbook 1 set

         * * In case the person is unable to come for registration, the Attorney shall prepare following document;

              - Copy of Identification Card of the Attorney 1 set

              - Letter of Power of Attorney 1 set

              - Copy of Krung Thai Bank’s Saving Account Passbook 1 set

2. Living Allowance for Persons with Disabilities 500 Bath per Month

         Qualification of Applicant for this living allowance are as below;

              - Hometown in area of allowance receiving application

         Please prepare below document for Persons with Disabilities Registration.

              - Copy of Identification Card/ Birth Certificate (in case of the child) 1 set

              - Copy of House Registration 1 set

              - Copy of The Disabled Book 1 set

              - Copy of Krung Thai Bank’s Saving Account Passbook 1 set

         * In case the person is unable to come for registration, the attorney shall prepare below document;

              - Copy of Attorney’s Identification Card 1 set

              - Letter of Power of Attorney 1 set

              - Copy of Krung Thai Bank’s Saving Account Passbook 1 set

3. Living Allowance for HIV Patients 500 Baht per month

         Qualifications of Applicant for this living allowance are as below;

              - Hometown in area of living allowance receiving application

         Please prepare below document for Persons with Disabilities Registration.

              - Copy of Identity Card/ Birth Certificate (in case of the child) 1 set

              - Copy of House Registration 1 set

              - Original Medical Certificate 1 set

              - Copy of Krung Thai Bank’s Saving Account Passbook 1 set

         * In case the person is unable to come for registration, the attorney shall prepare document below;

              - Copy of Attorney’s Identification Card 1 set

              - Letter of Power of Attorney 1 set

              - Copy of Krung Thai Bank’s Saving Account Passbook of the Attorney 1 set

4. Persons with Disabilities Registration

         Bring document to submit for registration at;

              1. Office of Chonburi Social Development and Human Security (City Hall)

              2. Chonburi Hospital

              3. Saensuk Municipality

         Please prepare below document for Persons with Disabilities Registration.

              - Copy of Identification Card/ Birth Certificate (in case of the child) 2 sets

              - Copy of House Registration 2 set

              - 2 pieces of 1 inch photo (taken within 6 months)

              - Disability Certification Document from Public Hospital Only

         * In case the person is unable to come for registration, the attorney shall prepare document below;

              - Copy of Attorney’s Identification Card 1 set

              - Copy of Attorney’s House Registration 1 set

              - Letter of Power of Attorney 1 set

5. Giving advices and contact related organizations where public service of social work and welfare are given.

         Procedure is as below;

              - Fill in application form for general support service.

              - Copy of Identification Card 1 set

              - Copy of House Registration 1 set

              - 1 piece of 1 inch photo

         * Municipality will give our support but in case the matter is beyond our power and responsibility, we will contact related organizations for further support.

6. Survey and Visit Houses of the Disadvantaged.
